Release, Waiver, and Agreement for Training Participation

Release and Waiver of Liability

In consideration of services received as a student on these premises, the undersigned hereby releases and forever discharges the school, its heirs, successors, administrators, and assigns from any and all actions, causes of action, liabilities, claims, and demands arising from any damage, loss, injury, or suffering—whether known or unknown—which may be sustained in connection with or during the course of receiving training and techniques on these premises. This release applies to incidents involving the instructor, instructor staff, officials, employees of the school, or any fellow students on the premises in connection with training or lessons detailed in this application.

Waiver of Claims

The undersigned further waives all rights to claims, actions, causes of action, damages, or suits for loss, injury, damage, or suffering sustained as a result of anything other than gross negligence on the part of the school. All inherent and incidental risks associated with participation in this type of sport activity are assumed by the undersigned as a condition for applying for admission to this school.

Authorization for Use of Images and Recordings

For the purposes stated above, the undersigned authorizes the school, its agents, successors, and assigns to photograph, video record, or use the voices, photographs, and images of participants without restriction. The school may utilize these photographs and voice transcripts for any commercial purposes, including but not limited to promotion or marketing of the school on the Internet. Participants agree that no compensation will be received as a result of such use.

Belt Recognition and Promotion

Official belt recognition and promotion will be issued upon completion of examination in consideration of the training obtained.

Non-Competition Agreement

The undersigned agrees not to engage in teaching martial arts or offer similar training or instruction within a 50-mile radius of any school operated by an official member of the Society of Harmonious Fist Federation, either during the continuance of this agreement or for a period of five years following termination of association with the Society, without express written authorization.

Acknowledgment and Compliance

The undersigned hereby acknowledges having read, understood, and agreed to comply with all of the above terms and conditions.
